Incretin-based therapies are a class of medications used to treat type 2 diabetes and obesity. They work by mimicking the actions of natural incretin hormones, such as glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP) which help regulate blood sugar levels. Medications like GLP-1/GIP agonists can be very helpful tools, but need to be part of a holistic approach to achieve sustained weight loss and health benefits. The key benefits of GLP-1 and dual GLP-1/GIP agonists for weight loss are their ability to suppress appetite, reduce food intake, and produce significant weight loss - especially when combined with lifestyle changes. However, they are not a standalone solution and work best as part of a comprehensive weight management program. GLP-1 agonists like semaglutide and tirzepatide (a dual GLP-1/GIP agonist) have been shown to provide significant weight loss benefits. They help suppress appetite and reduce food intake by:

  • Stimulating insulin secretion and suppressing glucagon, which regulates blood sugar levels

  • Delaying gastric emptying to increase feelings of fullness

  • They can produce profound weight loss, often in combination with diet and lifestyle changes

  • The dual GLP-1/GIP agonist tirzepatide appears to enhance the weight loss effects compared to GLP-1 agonists alone

The most successful outcomes occur when the medications are combined with a comprehensive weight management program, dietary changes, regular exercise and addressing underlying mental/physiological factors that contribute to obesity.

Obesity is a chronic condition, similar to hypertension and heart disease, and therefore can require medication maintenance to manage it.  Though GLP-1 agonists and GIP agonists are intended for long-term use to manage obesity as a chronic condition, patients may be able to maintain their weight loss even after discontinuing the medication, especially if they combine it with a comprehensive weight loss program and exercise. The duration of treatment needed to maintain weight loss can vary between individuals.

Who is a candidate for GLP-1 agonist and GLP-1 agonist and GIP agonist medication for weight loss?

Those with a Body Mass Index (BMI) > 30 kg/m2 or those with a BMI of > 27 kg/m2 with weight-related comorbidities.
